Individual Consultations

Health Management Dietetics provides you with the tools to effectively manage your health and reach your goals!

Medical nutrition therapy and nutrition counselling/coaching is implemented on a one-on-one face to face basis, via telephone or email. Initial consultations are between 40 and 60 minutes and follow-up consultations are between 20 and 30 minutes.

During consultations we will discuss your personal requirements and goals. You will have your body composition analysed if appropriate, and your current nutrition and lifestyle behaviours will be compared to optimal nutrition and lifestyle goals for your personal circumstances, medical conditions, medications and lifestyle.

You will be provided with practical and achievable strategies aimed at improving health and reaching your individual goals. Advice is personalised to fit your requirements and most importantly your current lifestyle and health goals.

Your Appointments

Your appointment is personalised and for that reason you will be recommended to attend follow up consultations determined by your dietitian and you at your initial consultations. Some clients may receive the information they require at their initial appointment, where most benefit from ongoing appointments between 1 and 4 weeks apart. Your requirements will be discussed with your dietitian at your initial consultation and your treatment plan will change along your journey to health.

Our dietitians aim to be on time, so we need your assistance, by also being on time. For consideration of those who have appointments following you we appreciate that you arrive on time and 5 — 10 minutes early if you are a new client, as we have some forms that we need you to complete.

Private Health Fund Rebates

• Individual Consultations are rebatable through most private health funds. The rebate will depend on your level of cover, please call your health fund to see what rebate you will be entitled to.
• Health Management Dietetics has HICAPs facilities, for some health funds you will be able to claim your rebate on the spot.

Medicare Rebates

• With some chronic conditions, your GP may choose to refer you to Health Management under a team care plan that where your appointment is rebateable through the medicare plus initiative.
• Health Management Dietetics does not bulk bill for these services, and you will need to pay for your consultation up front and take the receipt back to medicare for your rebate. Currently the rebate is $48.75 for each session.

Department of Veteran's Affairs (DVA)

• Health Management Dietetics is a provider for the Department of Veterans Affairs (DVA). If you have a Gold card or relevant condition under other card types, you will not have to pay for your visit to the dietitian. Our friendly dietitians are also available for home visits if required. If you are DVA eligible, please see your GP for a referral and relevant blood tests prior to seeing the dietitian.

• Home Visits (extra to consultation) — $30 (not applicable to DVA eligible clients)
• Concession available to aged pension and disability pension holders.
